Registered Nurse Overview

• The Registered Nurse will provide skilled hospice and home-based nursing care to patients in their homes.
• This role involves field-based nursing care within Bergen, Passaic, and Hudson Counties.
• Focus on individualized patient care, including assessment, planning, implementation, and evaluation.
• Collaborate with physicians, clinical leadership, and interdisciplinary teams for comprehensive care delivery.
• Ensure compliance with physician orders, insurance authorizations, and payor requirements.
• Provide education to patients and caregivers tailored to individual care goals.
• Maintain accurate clinical documentation and meet productivity standards.
• Work independently in a field-based environment while managing a patient caseload effectively.

Registered Nurse Key Responsibilities & Duties

• Perform comprehensive patient assessments and develop individualized care plans.
• Administer treatments and skilled nursing procedures as prescribed.
• Monitor patient conditions and communicate changes to physicians and clinical leadership.
• Conduct medication reconciliation and collaborate on treatment plans.
• Educate patients and caregivers on care goals and health management.
• Coordinate care with interdisciplinary team members and make appropriate referrals.
• Ensure compliance with insurance authorizations and documentation standards.
• Supervise home health aides and ensure quality care delivery.
• Communicate pertinent information during patient transfers and scheduled time off.

Registered Nurse Job Requirements

• Active NJ Registered Nurse License and AHA BLS/HCP Certification required.
• Associate's Degree in Nursing or Nursing Diploma; BSN preferred.
• Minimum 6+ months RN experience or 1+ year LPN experience.
• Home care or hospice experience strongly preferred.
• Proficiency in patient assessment, care planning, and medication management.
• Strong clinical judgment, documentation, and time-management skills.
• Comfortable traveling to patients' homes and working independently.
• Ability to lift, push, and pull up to 50 lbs and stand for extended periods.
• IV Certification and previous home care or hospice experience preferred.
