Job Description

Registered Nurse Overview

  • The Registered Nurse (RN) will provide and coordinate multidisciplinary health services for older adults in a dynamic Adult Day Care Center.
  • Collaborate with physicians and healthcare teams to implement comprehensive care plans tailored to patient needs.
  • Conduct clinical assessments and ensure accurate documentation of nursing evaluations and care plans.
  • Administer medications and provide skilled nursing services in compliance with physician orders.
  • Oversee and assign tasks to Certified Nursing Assistants (CNAs) based on established care plans.
  • Evaluate patient progress and communicate updates to the multidisciplinary team to ensure optimal care delivery.
  • Educate patients and families on disease management, self-care techniques, and preventive strategies.
  • Maintain compliance with healthcare regulations and standards while delivering compassionate care.
  • Participate in team meetings to discuss patient progress and adherence to care plans.

Registered Nurse Key Responsibilities & Duties

  • Conduct comprehensive clinical assessments and document findings in patient records quarterly and upon intake.
  • Ensure timely completion and follow-up of physician orders to maintain continuity of care.
  • Administer medications, prefill prescriptions, and perform skilled nursing tasks as prescribed.
  • Coordinate and oversee CNA tasks to ensure alignment with patient care plans.
  • Collaborate with healthcare teams to evaluate patient diagnoses and adjust interventions as necessary.
  • Monitor patient health status and communicate changes to physicians and team members promptly.
  • Educate patients and families on health management and preventive care strategies.
  • Participate in multidisciplinary meetings to report on patient progress and adherence to care plans.
  • Maintain accurate documentation of all services provided, ensuring compliance with healthcare standards.

Registered Nurse Job Requirements

  • Current license and registration to practice as a Registered Professional Nurse in New York State.
  • Bachelor of Science in Nursing (BSN) from an accredited institution preferred.
  • Minimum of one year of experience in acute or subacute care settings.
  • Experience in Adult Care Centers or geriatric care preferred.
  • Bilingual proficiency in Spanish and English is highly desirable.
  • UAS certification is preferred for enhanced patient assessment capabilities.
  • Strong clinical documentation and diagnosis coding skills.
  • Ability to collaborate effectively with multidisciplinary teams.
