Care Coordination Specialist Overview

  • The Care Coordination Specialist is a key member of the healthcare team, ensuring comprehensive medical case management services for all primary care patients.
  • Collaborate with clinical and support staff to manage patient enrollments, referrals, and administrative processes within the EMR system.
  • Support patients in navigating insurance programs, including ADAP applications and authorization processes.
  • Document hospitalizations, schedule discharge appointments, and monitor client progress to enhance patient outcomes.
  • Participate in morning huddles and medical staff meetings, contributing to quality improvement initiatives.
  • Coordinate referrals, ensuring proper documentation, insurance authorizations, and communication of outcomes to providers.
  • Oversee program enrollment and manage resource distribution, including gift card allocation and record-keeping.
  • Advocate for patients affected by homelessness, substance use, and HIV/AIDS, supporting the organization’s mission.

Care Coordination Specialist Key Responsibilities & Duties

  • Oversee the distribution of program resources, including gift cards, and maintain accurate records.
  • Provide coverage for Patient Access Specialists and support general administrative tasks as needed.
  • Advocate for patients by addressing challenges related to homelessness, substance use, and HIV/AIDS.

Care Coordination Specialist Job Requirements

  • High School Diploma or GED required; relevant certifications are a plus.
  • Minimum of 2 years of experience in a medical or related field preferred.
  • Proficiency in using Electronic Medical Record (EMR) systems for documentation and data entry.
  • Strong organizational skills and ability to manage multiple tasks in a fast-paced environment.
  • Excellent communication skills for collaborating with clinical teams and supporting patient care coordination.
  • Knowledge of insurance programs and processes, including ADAP, is highly desirable.
