Nurse Manager Case Management Overview

  • The Nurse Manager Case Management will oversee and coordinate case management operations within a healthcare organization, ensuring optimal patient care and resource utilization.
  • This role involves collaboration with multidisciplinary teams, including nurses, social workers, and physicians, to facilitate effective care transitions and patient advocacy.

Nurse Manager Case Management Key Responsibilities & Duties

  • Lead and manage a team of case managers, ensuring high-quality care coordination and adherence to best practices.
  • Develop and implement care management strategies to address patient needs and improve health outcomes.
  • Collaborate with interdisciplinary teams to facilitate effective communication and care transitions across the continuum of care.
  • Monitor and evaluate care plans to ensure alignment with patient goals and organizational standards.
  • Address social determinants of health and other barriers impacting patient care and recovery.
  • Ensure compliance with regulatory requirements and organizational policies related to case management practices.
  • Provide education and support to staff on case management processes and best practices.
  • Analyze data and metrics to identify trends and opportunities for improvement in care coordination and resource utilization.

Nurse Manager Case Management Job Requirements

  • Bachelor’s Degree in Nursing (BSN) required; Master’s Degree in Healthcare or related field preferred.
  • Active California Registered Nurse License upon hire is mandatory.
  • Minimum of 5 years of experience in a healthcare role, with 3 years in case management within an acute care setting.
  • At least 1 year of healthcare leadership experience is required.
  • Certified Case Manager (CCM) or Accredited Case Manager (ACM) certification preferred.
  • Strong knowledge of care coordination processes, social determinants of health, and resource management.
  • Excellent communication, leadership, and organizational skills to manage a multidisciplinary team effectively.
  • Ability to analyze data and implement strategies to improve patient outcomes and operational efficiency.
