Nurse Manager Case Management Key Responsibilities & Duties

Nurse Manager Case Management Job Requirements
- Bachelor’s Degree in Nursing (BSN) required; Master’s Degree in Healthcare or related field preferred.
- Active California Registered Nurse License upon hire is mandatory.
- Minimum of 5 years of experience in a healthcare role, with 3 years in case management within an acute care setting.
- At least 1 year of healthcare leadership experience is required.
- Certified Case Manager (CCM) or Accredited Case Manager (ACM) certification preferred.
- Strong knowledge of care coordination processes, social determinants of health, and resource management.
- Excellent communication, leadership, and organizational skills to manage a multidisciplinary team effectively.
