Homecare Nurse Key Responsibilities & Duties

• Provide in-home nursing care, including patient assessments and individualized care planning.
• Administer treatments, procedures, and medication reconciliation as per physician orders.
• Educate patients and families on health management and care practices.
• Monitor patient progress, adjusting care plans to meet evolving needs.
• Ensure accurate documentation and compliance with regulatory standards.
• Coordinate care with physicians and interdisciplinary teams to optimize patient outcomes.
• Supervise home health aides, conducting required supervisory visits.
• Perform specialized clinical tasks such as wound care, ostomy care, and catheter management.
• Communicate changes in patient conditions to appropriate parties promptly.

Homecare Nurse Job Requirements

• Active Registered Nurse (RN) license in New Jersey and AHA BLS certification.
• Associate’s Degree in Nursing required; Bachelor of Science in Nursing (BSN) preferred.
• Minimum of 6 months RN experience or 1 year LPN experience.
• Strong preference for candidates with home care, hospice, med-surg, or wound care backgrounds.
• Proficiency in clinical tasks such as ostomy care, wound care, and G-tube management.
• Basic computer skills and ability to document accurately and efficiently.
• Ability to lift, push, or pull up to 50 lbs and stand for extended periods.
• High level of independence and strong time management skills required.
• Preferred certifications include BSN and IV Certification.
