Job Description
Registered Nurse Overview
- The Registered Nurse will specialize in diabetes care and education, working closely with members to manage their health and improve outcomes.
- Provide hybrid work flexibility, alternating between in-office and remote work schedules.
- Collaborate with the Quality Assurance RN Manager to address gaps in diabetes care and support members with high A1Cs.
- Develop personalized management plans in partnership with members and their healthcare providers.
- Educate members on diabetes self-management, including nutrition, exercise, and medication adherence.
- Assist members in utilizing prescribed devices such as insulin pens and glucose monitors effectively.
- Promote healthier lifestyles to reduce complications and emergency care needs.
- Engage in quality assurance and improvement processes to enhance member care.
Registered Nurse Key Responsibilities & Duties
- Contact members and providers to create tailored diabetes management plans.
- Provide culturally sensitive diabetes education and support to members.
- Enable members to better understand blood glucose management and develop coping strategies.
- Identify patterns in self-management and suggest improvements for healthier living.
- Educate members on the use of diabetes-related devices and tools.
- Collaborate with physicians to align member care with medical advice.
- Participate in quality improvement initiatives to enhance service delivery.
- Perform additional duties and projects as assigned by management.
Registered Nurse Job Requirements
- Valid New York State Registered Nurse (RN) license is required.
- Bachelor’s degree in Nursing (BSN) is preferred.
- Certification as a Diabetes Care and Education Specialist is required.
- Minimum of three years of diabetes care experience and two years in nursing.
- Proficiency in clinical assessment, patient education, and motivational interviewing.
- Intermediate computer skills and familiarity with Microsoft Office applications.
- Bilingual proficiency in Spanish is preferred.
- Valid NYS Driver’s license and ability to travel within the metropolitan area.
