Behavioral Health Case Manager Overview

• The Behavioral Health Case Manager ensures high-quality behavioral health interventions for patients at the client’s primary care clinic.
• Provide proactive case management, comprehensive assessments, and individualized care plans to enhance patient access and retention in care.
• Coordinate crisis intervention, stabilization, and brief counseling services for patients as needed.
• Facilitate referrals to specialty care, entitlements, and long-term mental health services to support positive health outcomes.
• Collaborate with multidisciplinary teams to ensure seamless care coordination and communication.
• Maintain accurate and organized patient records to comply with documentation and reporting requirements.
• Promote services and recruit patients, focusing on underserved communities and individuals with low income.
• Participate in continuous quality improvement activities and professional development opportunities.

Behavioral Health Case Manager Key Responsibilities & Duties

• Conduct assessments, reassessments, and care plan development to ensure effective case management services.
• Provide crisis intervention and brief counseling to address immediate patient needs.
• Coordinate referrals to internal and external providers and follow up on outcomes.
• Act as a liaison between patients and multidisciplinary teams to enhance care coordination.
• Respond to inquiries regarding services and provide timely information.
• Maintain compliance with organizational policies and local, state, and federal regulations.
• Participate in internal and external meetings, trainings, and community events as needed.
• Ensure accurate documentation in electronic and paper records for reporting purposes.

Behavioral Health Case Manager Job Requirements

• Master’s degree in Social Work or Mental Health Counseling required.
• Minimum of two years of relevant work experience, including internships and volunteer work.
• Licensure as LMHC or LMSW required within three months of hire.
• UAS-NY certification required within one month of hire.
• Experience working with diverse populations, including individuals with serious mental illness and substance use disorders.
• Proficiency in various software applications such as Outlook, Word, and Excel.
• Fluency in Spanish or Asian languages is a plus.
• Ability to work flexible hours, including evenings and weekends.
