Case Management Supervisor Overview

  • The Case Management Supervisor oversees healthcare case management staff to ensure high-quality patient care and optimal health outcomes.
  • Collaborates with multidisciplinary teams to monitor and evaluate patient progress for positive health results.
  • Provides direct supervision, training, and coaching to case management staff, ensuring adherence to policies and regulations.
  • Develops and maintains patient-focused care plans in collaboration with care teams and patients.
  • Ensures accurate documentation of patient interactions and services in designated databases.
  • Participates in quality assurance and improvement activities to enhance service delivery.
  • Represents the organization at external venues to promote services and foster partnerships.
  • Responds to inquiries from external entities and facilitates referrals to agency services.

Case Management Supervisor Key Responsibilities & Duties

Case Management Supervisor Job Requirements

  • Bachelor’s degree in health, nursing, mental health, or related field with 4 years of experience.
  • Advanced degree preferred with 2 years of relevant experience.
  • Supervisory experience and familiarity with target populations strongly preferred.
  • UAS-NY certification required within one month of hire.
  • Proficiency in various applications such as Outlook, Word, Excel, and healthcare databases.
  • Excellent written and verbal communication skills in English required.
  • Ability to multitask and manage projects in a high-pressure environment.
