• The Case Manager – Medical will provide direct case management to patients, ensuring access to and retention in medical care.
• Collaborate with a multidisciplinary team to deliver comprehensive care and support services.
• Coordinate patient access to specialty care, entitlements, mental health, and substance use treatment.
• Develop individualized care plans based on thorough assessments and reassessments.
• Ensure accurate documentation and reporting to meet agency requirements.
• Recruit and support specific communities, including individuals living with HIV/AIDS and underserved populations.
• Utilize electronic medical records and office software for efficient case management.
• Contribute to continuous quality improvement activities within the clinic.
• Adhere to health and safety certifications as required by regulatory bodies.

Case Manager – Medical Key Responsibilities & Duties

• Conduct patient intake, assessments, and care plan development to ensure access to medical care.
• Coordinate with primary care providers and multidisciplinary teams for optimal patient outcomes.
• Link patients to necessary services and report outcomes to care teams.
• Obtain prior authorizations for pharmacy or medical services as needed.
• Maintain accurate electronic and paper records for compliance and reporting.
• Recruit underserved populations to facilitate early access to treatment and services.
• Participate in clinic quality improvement activities and initiatives.
• Perform other duties as assigned to support clinic operations and patient care.
• Ensure compliance with health and safety regulations and certifications.

Case Manager – Medical Job Requirements

• Bachelor’s degree in social science, psychology, social work, or health-related field required.
• Minimum of three years relevant experience in case management or related field.
• Experience working with diverse communities, including individuals living with HIV/AIDS.
• Fluency in one or more Asian languages preferred.
• Proficiency in electronic medical records and common office software required.
• Excellent communication, organizational, and interpersonal skills essential.
• Ability to handle multiple tasks and work in a fast-paced environment.
• Flexibility to work evenings and weekends as needed.
• Proof of immunization and compliance with health codes required.
