Field Registered Nurse Overview

• The Field Registered Nurse provides comprehensive care to clients under the supervision of the RN Supervisor, ensuring compliance with healthcare regulations and organizational policies.
• Coordinate resources and services for clients based on health conditions, collaborating with families, physicians, and care providers to optimize outcomes.
• Conduct comprehensive client assessments to develop and implement effective care plans, ensuring timely home visits and adherence to regulatory requirements.
• Deliver evidence-based nursing care, including ongoing assessment, teaching, and training to patients and caregivers, preserving patient autonomy and dignity.
• Supervise Home Health Aides, assigning duties based on client needs and providing in-service training as required.
• Participate in quality improvement initiatives, contributing to organizational goals and enhancing patient satisfaction and care outcomes.
• Travel to various neighborhoods within Brooklyn, Queens, Manhattan, and occasionally the Bronx to deliver client care.
• Maintain accurate and timely documentation of patient care activities, ensuring compliance with regulatory and organizational standards.
• Engage in special projects and other related duties as assigned to support organizational objectives.

Field Registered Nurse Key Responsibilities & Duties

• Coordinate resources and services for assigned caseloads, ensuring optimal care delivery and collaboration with internal and external partners.
• Conduct comprehensive assessments to develop and implement care plans tailored to client needs and regulatory requirements.
• Provide ongoing education and training to patients and caregivers, promoting evidence-based practices and preserving patient dignity.
• Supervise Home Health Aides, assigning tasks based on client needs and conducting training sessions as required.
• Maintain accurate and timely documentation of care activities, ensuring compliance with organizational and regulatory standards.
• Respond to emergencies and acute changes in patient conditions, initiating appropriate interventions and referrals.
• Participate in quality improvement initiatives and meetings to enhance care outcomes and patient satisfaction.
• Travel to client homes across designated neighborhoods to deliver care and conduct assessments.
• Engage in special projects and other duties as assigned to support organizational goals.

Field Registered Nurse Job Requirements

• Bachelor of Science in Nursing (BSN) and current New York State RN license required.
• Minimum of two years of experience in home health care preferred; experience with certified home health agencies strongly preferred.
• Basic Cardiac Life Support certification required.
• Demonstrated knowledge of healthcare practices and ability to work independently and collaboratively.
• Strong organizational skills, attention to detail, and ability to travel across designated neighborhoods.
• Excellent verbal and written communication skills combined with tact and analytic judgment.
• Commitment to serving underserved communities and promoting equity and dignity in healthcare.
• Ability to understand and address institutional and structural racism and bias in healthcare delivery.
• Proficiency in Spanish and ability to navigate diverse cultural contexts preferred.
