Nurse Practitioner Overview

  • As a Nurse Practitioner, deliver patient-centered primary care to diverse populations.
  • Provide compassionate and unbiased care adhering to evidence-based guidelines.

  • Offer specialized care in HIV and transgender health, enhancing patient outcomes.

Nurse Practitioner Key Responsibilities & Duties

  • Conduct comprehensive patient screenings, examinations, and diagnoses across the lifespan.
  • Develop and implement treatment plans tailored to individual patient needs.
  • Provide timely follow-up on test results, referrals, and patient inquiries.
  • Collaborate with healthcare professionals to deliver coordinated, team-based care.
  • Participate in case conferences and quality improvement initiatives.
  • Ensure compliance with regulatory standards and organizational policies.

Nurse Practitioner Job Requirements

  • New York State Certification/Licensure as Nurse Practitioner with current registration.
  • Minimum of 2 years of primary care experience; 3 years preferred.
  • Experience in HIV and transgender care is highly desirable.
  • Proficiency in electronic health records (EPIC) and documentation standards.
  • Current CPR certification and DEA Controlled Substances Registration Certificate.
  • Strong collaboration skills within a team-based care model.
  • Commitment to accessible, patient-centered healthcare for diverse populations.
  • Bilingual proficiency in Spanish/English is a plus.
