Summary:
The Travel/Interim Licensed Clinical Social Worker is a temporary position that involves providing social work services on a short-term basis in healthcare settings such as hospitals, clinics, or rehabilitation centers. The primary role of the Travel/Interim LCSW is to assess the psychosocial needs of patients, provide counseling and support, and coordinate appropriate resources and services. This position requires flexibility, adaptability, and the ability to work effectively in different healthcare environments.
Responsibilities:
- Conduct comprehensive psychosocial assessments of patients to identify their social, emotional, and environmental needs.
- Provide counseling and emotional support to patients and their families, addressing issues such as adjustment to illness, grief, and coping strategies.
- Develop individualized care plans based on the assessment and in collaboration with the interdisciplinary team.
- Collaborate with the healthcare team to develop and implement discharge plans that address patients' social and support needs.
- Identify and coordinate appropriate community resources, such as home health services, transportation, and financial assistance programs.
- Assist patients and their families in accessing and navigating healthcare and social service systems.
- Respond to crisis situations, including mental health emergencies, abuse or neglect concerns, and end-of-life issues.
- Advocate for patients' rights and ensure their access to necessary services and support.
- Collaborate with community agencies and organizations to facilitate continuity of care for patients.
- Maintain accurate and timely documentation of assessments, interventions, and care plans.
- Communicate effectively with the healthcare team, patients, families, and external stakeholders.
- Participate in interdisciplinary meetings and provide input on patients' psychosocial needs and progress.
- Provide education to patients, families, and caregivers on available resources, support groups, and self-care strategies.
- Facilitate support groups or educational sessions as needed.
- Serve as a resource and consultant for healthcare professionals regarding social work issues and psychosocial interventions.
Qualifications:
- Master's degree in Social Work (MSW) from an accredited program.
- Current licensure or certification as a social worker (LCSW, LMSW, or equivalent) in the respective jurisdiction.
- Previous experience in medical or healthcare social work, preferably in a variety of settings.
- Strong knowledge of psychosocial assessment, crisis intervention, and discharge planning.
- Familiarity with community resources and social service programs.
- Excellent communication, interpersonal, and advocacy skills.
- Ability to work independently and adapt to different work environments.
- Strong organizational and time management abilities.
- Willingness to travel and work on a temporary/interim basis.
