Job Description
RESPONSIBILITIES:
1. Makes initial contact (physical, social and psychological) to determine appropriateness of hospice services.
2. Works with referral partners to provide appropriate alternatives for patients who do not meet criteria including patients with no funding source.
3. Assists with patient’s insurance to determine coverage.
4. Ensures accurate authorizations for disciplines.
5. Self identifies options for payment, which are not necessarily identified by patient.
6. Determines needs and coping skills of caregiver in caring for patient.
7. Interprets policies and acts as resource for case managers, physicians, hospital and community for effective public relations and marketing of services.
8. Obtain and enter verbal certification into EMR from primary provider prior to Hospice Admission.
9. Initiates referral-to-admission process for each patient referred by facilitating required data, leveraging available resources to ensure data is complete and accurate.
QUALIFICATIONS:
1. Graduate RN from accredited school of nursing, BSN preferred
2. Current license to practice profession in the state where employee is providing service.
3. Current driver’s license in state of residence. Possession of a registered and insured motor vehicle.
4. Two years of experience working in an acute care setting; one year of home care and/or hospice experience preferred.
5. Working knowledge of community resources.
6. Ability to meet, collaborate and work efficiently with others in the community.
7. Knowledge of entitlement programs.
