    We are looking for an experienced Care Coordinator RN with recent experience in a California Dignity Health facility. This is a 10 hour day shift (varied days).

Job Title: Care Coordinator (RN)

Position Summary:

The Care Coordinator RN is responsible for ensuring the appropriate progression of care and discharge planning for patients in an acute hospital setting. This role requires clinical expertise, strong communication, and coordination skills to ensure optimal patient outcomes and cost-effective care delivery.

Qualifications:

  • Licensure: Active Registered Nurse (R.N.) license in the applicable state(s)
  • Certifications:
    • BLS certification (required if working in a hospital setting)
    • CCM, ACM-RN, or Utilization Management certification (preferred)
  • Education:
    • BSN preferred or equivalent experience

Experience Requirements:

  • Minimum 2 years of recent experience in Care Coordination, Case Management, or Care Management within an acute care hospital setting
  • Minimum 2 years of acute care bedside nursing experience
  • Prior experience in either a clinical or insurance/utilization management setting

Key Skills and Competencies:

  • Strong ability to apply clinical guidelines to support appropriate progression of care
  • In-depth understanding of managed care and payer environments
  • Effective critical thinking and problem-solving skills
  • Excellent professional communication and interpersonal skills
  • Working knowledge of how utilization management and case management integrate in patient care
  • Ability to work collaboratively as a team player and support colleagues when needed

  • Understanding of CMS standards and regulatory requirements
  • Strong skills in prioritization and delegation
