Job Description
Responsibilities:
- Collaborate with the member and/or family member to maintain the most independent living situation possible
- Ensure consistent care along the entire health care continuum by assessing and closely monitoring member’s needs and status. Responsible to authorize covered services and coordinate care regardless of payer.
- Collaborate and communicate with member/family/caregivers, primary care practitioners, and the interdisciplinary team
Qualifications:
- Bachelor’s degree in Nursing
- At least one year case management experience
- Knowledge of MLTC program requirements required
