  • Join as a Contract Registered Nurse in an acute hospital setting.
  • Provide care coordination and case management to optimize patient outcomes.

Key Responsibilities & Duties

  • Coordinate patient care plans ensuring effective transitions across settings.
  • Collaborate with healthcare professionals to optimize patient care outcomes.
  • Conduct utilization reviews to align with managed care requirements.
  • Educate patients and families about care plans and available resources.
  • Apply clinical guidelines for care progression and patient advocacy.
  • Ensure compliance with CMS standards and regulatory requirements.

Job Requirements

  • Current Registered Nurse (RN) license in applicable state(s).
  • Associate of Applied Science (AAS) in Nursing required.
  • Minimum 2 years of experience in acute hospital care coordination.
  • BLS certification required for hospital settings.
  • Preferred certifications: CCM, ACM-RN, or UM Certification.
  • Strong understanding of managed care and payer environments.
  • Excellent communication, critical thinking, and problem-solving skills.
