Overview
- Join a dedicated team providing essential nursing care to behavioral health clients in the community.
- Work closely with interdisciplinary teams to deliver comprehensive health services.
- Conduct initial assessments and ongoing health monitoring for clients in the ACT program.
- Administer medications and ensure compliance with prescribed treatments.
- Coordinate care with external medical providers and community organizations.
- Provide health education and training to clients and staff to promote wellness.
- Lead interdisciplinary team meetings and support program compliance initiatives.
- Supervise part-time nursing staff and oversee their assignments and training.
- Participate in emergency coverage rotations to ensure client safety.
Key Responsibilities & Duties
- Carry a caseload and conduct weekly visits to clients in shelters or homes.
- Develop and implement treatment plans tailored to individual client needs.
- Administer prescribed medical treatments and monitor for adverse reactions.
- Coordinate medication delivery and manage pharmacy interactions for clients.
- Provide advocacy and follow-up care for hospitalized clients.
- Facilitate health education groups and train staff on health practices.
- Audit program charts monthly to ensure compliance with standards of care.
- Act as a liaison between medical providers and the ACT program team.
- Respond to medical emergencies and document care provided accurately.
Job Requirements
- Bachelor of Science in Nursing (BSN) and New York State RN registration required.
- Minimum of 1 year of nursing experience, preferably in behavioral health settings.
- Strong knowledge of medication administration and health monitoring practices.
- Ability to work effectively in a field-based, community-focused role.
- Experience coordinating care with external providers and community organizations.
- Proficiency in documentation and compliance with regulatory standards.
- Capability to supervise and train part-time nursing staff.
- Flexibility to provide after-hours emergency coverage on a rotating basis.
