Overview

- Serve as a Registered Nurse, managing client health needs and collaborating with multidisciplinary teams to optimize treatment outcomes.

Key Responsibilities & Duties
- Act as primary medical case manager, conducting health checks and coordinating with nursing leadership and team members.
- Link clients' medical and psychosocial needs, making referrals and suggestions for medical and psychiatric treatment.
- Document client health findings, maintain medication administration records, and ensure adequate medication supply.
- Provide health education to clients, advocating for their preferences and developing health goals collaboratively.
- Participate in treatment team meetings and case conferences to share client needs and optimize care plans.
- Deliver emergency first aid and CPR assistance as required, ensuring immediate client safety.
- Collaborate with physicians, pharmacies, and other treatment collaterals to coordinate comprehensive care.
- Perform related duties as assigned to support the overall mission of the healthcare facility.
Job Requirements
- Active Registered Nurse (RN) license required; Bachelor of Science in Nursing (BSN) preferred.
- Minimum of 1 year of nursing experience; 3 years preferred for optimal performance.
- Proficiency in medical case management and health education delivery to diverse client populations.
- Strong documentation skills to maintain medication records and controlled substance logs.
- Ability to collaborate effectively with multidisciplinary teams and external healthcare providers.
- Knowledge of emergency care procedures, including first aid and CPR.
- Commitment to client advocacy and development of personalized health goals.
- Availability to work onsite during scheduled hours, including weekends.
