Job Description
Overview
- Provide comprehensive case management services to patients in a healthcare setting.
- Utilize expertise in social work to address patient needs and coordinate care plans.
- Collaborate with interdisciplinary teams to ensure optimal patient outcomes.
- Support patients experiencing homelessness, mental health crises, or substance abuse challenges.
- Organize and participate in family meetings to facilitate care decisions.
- Assist in arranging durable medical equipment, skilled nursing facilities, and transportation services.
- Contribute to the development and implementation of patient treatment plans.
- Ensure compliance with healthcare regulations and organizational policies.
Key Responsibilities & Duties
- Conduct patient assessments to identify social, emotional, and medical needs.
- Develop and implement individualized care plans in collaboration with healthcare teams.
- Provide counseling and support to patients and families during challenging situations.
- Coordinate with external agencies for protective services and community resources.
- Educate patients and families on advance directives and POLST forms.

Job Requirements
- Master’s degree in Social Work from an accredited institution.
- Minimum of 2 years of experience in a healthcare or social services setting.
- Proficiency in case management and patient advocacy practices.
- Knowledge of healthcare systems, HMOs, and community resources.
- Ability to assess and address patient needs effectively and independently.
- Strong organizational skills and ability to manage multiple cases simultaneously.
- Familiarity with protective services processes and advance directive protocols.
- Excellent communication and interpersonal skills for patient and team interactions.
