Overview

Key Responsibilities & Duties
- Conduct assessments to determine member needs and develop personalized care plans.
- Monitor member progress and adjust care plans as necessary to achieve optimal outcomes.

- Authorize services and manage care transitions effectively for members.

Job Requirements
- Bachelor of Science in Nursing (BSN) and current NYS RN License and registration.
- Minimum three years of RN experience, preferably in geriatrics or managed long-term care.
- Proficiency in bilingual communication, particularly in Chinese, is highly desirable.
- Strong clinical expertise and ability to assess and monitor member needs effectively.
- Experience in coordinating care and authorizing services across diverse healthcare settings.
- Ability to work in a hybrid environment with fieldwork and remote responsibilities.
