Job Description

Overview

  • Serve as a Nurse Case Manager in a hybrid role, balancing remote and fieldwork responsibilities.
  • Collaborate with members and families to ensure independent living and consistent care across the healthcare continuum.
  • Assess and monitor members’ needs and statuses to provide effective healthcare solutions.
  • Authorize covered services and coordinate care irrespective of payer sources.
  • Engage with interdisciplinary teams to optimize care delivery for members.
  • Work in a bilingual environment, supporting diverse member needs.
  • Enjoy a hybrid work schedule with three remote days and two fieldwork days weekly.

Key Responsibilities & Duties

  • Conduct assessments to determine member needs and develop personalized care plans.
  • Monitor member progress and adjust care plans as necessary to achieve optimal outcomes.
  • Coordinate with primary care practitioners and interdisciplinary teams to ensure seamless care delivery.
  • Authorize services and manage care transitions effectively for members.
  • Communicate with members, families, and caregivers to address concerns and provide support.
  • Ensure compliance with healthcare regulations and organizational policies.
  • Utilize bilingual skills to enhance communication and care delivery.
  • Participate in training and development activities to maintain clinical expertise.

Job Requirements

  • Bachelor of Science in Nursing (BSN) and current NYS RN License and registration.
  • Minimum three years of RN experience, preferably in geriatrics or managed long-term care.
  • Proficiency in bilingual communication, particularly in Chinese, is highly desirable.
  • Strong clinical expertise and ability to assess and monitor member needs effectively.
  • Experience in coordinating care and authorizing services across diverse healthcare settings.
  • Ability to work in a hybrid environment with fieldwork and remote responsibilities.
  • Excellent communication and collaboration skills with interdisciplinary teams and families.
