Job Description

Overview

  • Serve as an RN Case Manager on a 13-week contract at a leading healthcare organization.
  • Provide comprehensive care coordination and management for patients during day shifts.
  • Collaborate with interdisciplinary teams to ensure optimal patient outcomes and satisfaction.
  • Utilize clinical expertise to assess, plan, implement, and evaluate patient care plans.
  • Work onsite in a dynamic and supportive hospital environment.

Key Responsibilities & Duties

  • Conduct patient assessments to determine healthcare needs and coordinate appropriate services.
  • Develop and implement individualized care plans in collaboration with healthcare teams.
  • Monitor patient progress and adjust care plans as necessary to achieve desired outcomes.
  • Educate patients and families about healthcare options and resources available.
  • Ensure compliance with hospital policies and regulatory standards.
  • Participate in case reviews and interdisciplinary team meetings to optimize patient care.
  • Maintain accurate and timely documentation of patient interactions and care plans.

Job Requirements

  • Associate of Applied Science (AAS) in Nursing or equivalent required.
  • Minimum of 2 years of experience in case management or related nursing roles.
  • Current RN licensure and certifications as per state requirements.
  • Strong knowledge of care coordination and healthcare management practices.
  • Proficiency in electronic medical records (EMR) systems and documentation.
  • Ability to work effectively in an onsite hospital setting during day shifts.
  • Excellent communication and interpersonal skills to collaborate with teams and patients.
