Overview
- Serve as an RN Case Manager for hospice care, providing compassionate support to patients in Brooklyn.
- Conduct physical and psychosocial assessments for patients in end-stage life conditions.
- Develop and implement personalized nursing care plans based on patient needs.
- Supervise and evaluate home health aides and personal care workers in patient homes.
- Coordinate with physicians to adjust care plans as necessary.

Key Responsibilities & Duties
- Perform comprehensive assessments to evaluate physical and psychosocial health of hospice patients.
- Develop and execute individualized nursing care plans addressing patient needs.

- Collaborate with physicians to review and modify care plans as required.

Job Requirements
- Associate of Science (AS) degree in Nursing is required.
- Active RN certification with a minimum of 2 years of clinical experience.
- Experience in hospice care preferred; home care or acute care experience considered.
- Proficiency in clinical data entry and documentation.
- Strong communication, teaching, and interpersonal skills.
- Ability to work effectively within interdisciplinary teams.
- Field-based role requiring 100% work in Brooklyn, including rotating weekends.
