Job Description

Overview

  • Serve as an RN Case Manager for hospice care, providing compassionate support to patients in Brooklyn.
  • Conduct physical and psychosocial assessments for patients in end-stage life conditions.
  • Develop and implement personalized nursing care plans based on patient needs.
  • Supervise and evaluate home health aides and personal care workers in patient homes.
  • Coordinate with physicians to adjust care plans as necessary.
  • Provide hands-on care and collaborate within interdisciplinary team environments.
  • Work full-time field-based hours, including rotating weekends, in Brooklyn.
  • Receive training and professional development opportunities to enhance skills.

Key Responsibilities & Duties

  • Perform comprehensive assessments to evaluate physical and psychosocial health of hospice patients.
  • Develop and execute individualized nursing care plans addressing patient needs.
  • Supervise and train home health aides, ensuring quality patient care delivery.
  • Collaborate with physicians to review and modify care plans as required.
  • Document clinical data accurately and maintain patient records.
  • Provide education and guidance to patients and families regarding hospice care.
  • Ensure compliance with healthcare regulations and standards.
  • Participate in interdisciplinary team meetings to optimize patient care strategies.

Job Requirements

  • Associate of Science (AS) degree in Nursing is required.
  • Active RN certification with a minimum of 2 years of clinical experience.
  • Experience in hospice care preferred; home care or acute care experience considered.
  • Proficiency in clinical data entry and documentation.
  • Strong communication, teaching, and interpersonal skills.
  • Ability to work effectively within interdisciplinary teams.
  • Field-based role requiring 100% work in Brooklyn, including rotating weekends.
