Job Description

Overview

  • Provide weekend on-call triage nursing services, addressing urgent patient needs remotely and in the field as required.
  • Coordinate with the Call Center Triage and On-Call Manager to ensure effective patient care during after-hours shifts.
  • Offer emotional support and guidance to families during patient deaths, ensuring compassionate care.
  • Educate patients and caregivers on care plans, medication management, and symptom management.
  • Work remotely with occasional field visits, utilizing transportation services as needed.
  • Flexible shift options, including the ability to work double shifts.
  • Adhere to New York State Standards for Professional Registered Nurses in all aspects of care.

Key Responsibilities & Duties

  • Respond promptly to triage visit requests via cell phone, ensuring effective communication with the on-call team.
  • Implement care plans, addressing urgent physical and psycho-social issues for patients during shifts.
  • Coordinate with Hospice physicians to establish safe and effective care plans for patients.
  • Provide education to patients and caregivers regarding changes to care plans and medication management.
  • Attend patient deaths, offering emotional support and guidance to families as needed.
  • Report patient-related visits and care plan changes to triage and primary Hospice teams.
  • Utilize Word, Excel, and Windows applications for documentation and communication purposes.

Job Requirements

  • Associate of Science (AS) degree in Nursing from an accredited institution.
  • Minimum of 2 years of experience in Medical-Surgical, Community Health, Hospice, or Oncology nursing.
  • Preferred 4 years of relevant nursing experience.
  • Valid New York State RN licensure and NYS driver's license preferred.
  • Commitment to obtaining national board certification CHPN within one year of hire.
  • Proficiency in using Word, Excel, and Windows applications for documentation and communication.
  • Ability to work remotely with occasional field visits, utilizing transportation services.
