Job Description

Overview

  • Provide professional rehabilitation therapy to patients in their homes, ensuring personalized care and support.
  • Develop and implement goal-oriented therapy programs tailored to individual patient needs.
  • Conduct comprehensive evaluations to assess patient conditions and progress.
  • Collaborate with healthcare teams to ensure effective coordination and resource utilization.
  • Provide education to patients and caregivers for effective home program implementation.
  • Maintain accurate and timely documentation of patient progress and care plans.
  • Supervise and assess the performance of Physical Therapy Assistants.
  • Ensure compliance with organizational policies and external regulations.
  • Participate in discharge planning and provide recommendations for continued care.

Key Responsibilities & Duties

  • Perform patient evaluations to develop tailored therapy programs with measurable goals.
  • Document patient progress, reassessments, and discharge summaries accurately and promptly.
  • Coordinate care with physicians and healthcare teams to optimize patient outcomes.
  • Educate patients and caregivers on home therapy programs and health practices.
  • Supervise Physical Therapy Assistants, ensuring adherence to treatment plans.
  • Ensure therapy services align with physician's care plans and organizational standards.
  • Participate in discharge planning and provide recommendations for ongoing care.
  • Maintain compliance with healthcare regulations and organizational policies.
  • Respond to patient inquiries and coordinate schedules effectively.

Job Requirements

  • Doctoral degree in Physical Therapy (PT) or equivalent qualification.
  • Minimum of 1 year of professional experience in physical therapy; 3 years preferred.
  • Valid Physical Therapist License and AHA Basic Health Care Life Support HCP Certification.
  • Proficiency in using personal computers and relevant software applications.
  • Strong knowledge of rehabilitation therapy practices and patient care standards.
  • Ability to lift, push, and pull a minimum of 50 lbs and stand for extended periods.
  • Excellent communication skills for patient education and team collaboration.
  • Capability to develop and implement individualized treatment plans effectively.
  • Commitment to maintaining accurate and timely documentation of patient care.
