Overview
- Serve as a Case Manager, ensuring optimal care coordination and member independence within a hybrid work environment.
- Collaborate with members, families, and interdisciplinary teams to provide comprehensive healthcare solutions.
- Perform fieldwork two days per week within New York City, supporting diverse populations.
- Utilize bilingual skills to effectively communicate with Chinese and Spanish-speaking members.
- Ensure consistent care across the healthcare continuum by assessing and monitoring members' needs.
- Authorize covered services and coordinate care regardless of payer type.
- Work in a hybrid schedule: three days from home and two days in the field.
- Contribute to maintaining members' independence and quality of life.
- Participate in virtual and in-person interviews as part of the hiring process.
Key Responsibilities & Duties
- Assess members' healthcare needs and develop personalized care plans.
- Coordinate with primary care practitioners and interdisciplinary teams for effective care delivery.
- Monitor members' health status and adjust care plans as necessary.
- Provide bilingual support to Chinese and Spanish-speaking members.

Job Requirements
- Bachelor of Science in Nursing (BSN) and current NYS RN license required.
- Minimum three years of RN experience, preferably in geriatrics or managed long-term care.
- Bilingual proficiency in Chinese and Spanish preferred.
- Strong clinical expertise in geriatric care and healthcare coordination.
- Ability to work in a hybrid schedule and perform fieldwork in NYC.
- Excellent communication and collaboration skills with members and interdisciplinary teams.
