Job Description

Overview

  • Lead clinical operations within a residential treatment facility serving youth aged 12-18 with complex mental health diagnoses.
  • Collaborate with interdisciplinary teams to ensure therapeutic and educational success for residents.
  • Oversee transitional and permanency programs aimed at reducing length of stay and optimizing outcomes.
  • Provide clinical supervision to teams, focusing on family dynamics and socialization needs.
  • Establish collaborative relationships with local resources and community programs.
  • Ensure compliance with regulatory guidelines and maintain high standards of care.
  • Participate in treatment meetings and administrative updates to enhance program effectiveness.
  • Support discharge planning and aftercare services for residents transitioning to community settings.

Key Responsibilities & Duties

  • Develop and implement structures for transitional and permanency programs within the facility.
  • Supervise and support teams in assessing family strengths and needs, creating plans for reunification.
  • Lead joint meetings with community-based programs to ensure efficient family support.
  • Coordinate intensive community-based work to facilitate successful transitions for residents.
  • Monitor utilization of transitional funds in compliance with policies.
  • Provide direct services to a caseload as necessary and during vacancies.
  • Conduct chart reviews and community visits to ensure quality standards are met.
  • Serve as a critical member of the management team, contributing to strategic planning.

Job Requirements

  • Master's degree in Social Work or related field required.
  • Licensed Clinical Social Worker (LCSW) certification preferred.
  • Minimum of 2 years of experience in direct services for children with SED.
  • At least 2 years of supervisory or managerial experience in social services.
  • Experience in residential settings and working with high-acuity populations.
  • Proficiency in EMR systems and documentation standards.
  • Strong communication skills and ability to travel to family homes.
