Nurse Manager Home Care

Overview

• Lead and manage high-quality home care services delivery, ensuring compliance with agency standards and regulations.
• Provide leadership and guidance to nursing staff, fostering professional growth and development.
• Ensure adequate staffing levels and appropriate patient assignments to maintain care quality.
• Collaborate with interdisciplinary teams to achieve clinical, fiscal, and patient satisfaction goals.
• Evaluate and mentor clinicians to enhance performance and adherence to care standards.
• Oversee scheduling and documentation processes to ensure continuity and quality of care.
• Monitor and act on performance reports to improve adherence to workflows and guidelines.
• Support on-call staff in addressing after-hours inquiries and patient needs.
• Facilitate timely processing of home health care orders to ensure regulatory compliance.

Key Responsibilities & Duties

• Mentor nursing staff in delivering patient care aligned with agency policies and regulations.
• Identify and address educational opportunities to enhance clinician development.
• Evaluate individual and team performance to improve patient care and experience scores.
• Develop action plans to address performance gaps and monitor progress.
• Provide oversight for scheduling patient care services to ensure productivity and quality.
• Review reports to ensure adherence to workflows and address variances promptly.
• Conduct annual performance evaluations based on supervisory visits and clinician assessments.
• Support on-call staff in handling inquiries and providing clinical assistance.
• Ensure timely and compliant processing of home health care orders.

Job Requirements

• Bachelor of Science in Nursing (BSN) required; RN licensure mandatory.
• Minimum of 3 years’ experience in a Certified Home Health Agency; supervisory experience preferred.
• Proficiency in OASIS E, clinical outcome measures, and STAR ratings.
• Knowledge of Federal Conditions of Participation and NYS DOH regulations for home health care.
• Strong leadership and mentoring skills to guide nursing staff effectively.
• Ability to evaluate and improve clinician performance and adherence to care standards.
• Experience in managing interdisciplinary teams and achieving organizational goals.
• Excellent organizational and communication skills for effective team collaboration.
