Key Responsibilities & Duties

  • Conduct thorough assessments, diagnoses, and treatments for patients with acute and chronic conditions.
  • Develop and implement personalized care plans to address individual patient needs.
  • Lead and collaborate with care teams to ensure high-quality service delivery.
  • Engage in proactive patient education and health promotion activities.
  • Monitor and manage patient health outcomes using evidence-based practices.

Job Requirements

  • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree required.
  • Board certification in Internal Medicine, Family Medicine, or Geriatrics mandatory.
  • Active state medical license or eligibility to obtain one within the required timeframe.
  • Minimum of 1 year of clinical experience; 5 years preferred.
  • Proficiency in electronic medical records and healthcare technology systems.
  • Strong commitment to patient-centered care and value-based healthcare models.
  • Basic Life Support (BLS) certification required within the first 90 days of employment.
  • Excellent communication and interpersonal skills to engage effectively with patients and teams.
