Job Description

Overview

  • Provide comprehensive primary care services to patients, focusing on holistic and value-based care models.
  • Collaborate with multidisciplinary teams to ensure optimal patient outcomes and satisfaction.
  • Maintain a manageable patient panel size to deliver personalized and effective care.

Key Responsibilities & Duties

  • Diagnose, treat, and manage acute and chronic illnesses in adult patients.
  • Lead care teams to deliver high-quality, patient-centered medical services.
  • Ensure compliance with clinical guidelines and quality standards.
  • Actively participate in risk and quality management programs.
  • Engage with hospitalists for continuity of care during hospital admissions.
  • Educate patients on health promotion and disease prevention strategies.
  • Document patient care activities accurately and efficiently.
  • Recruit patients and providers to support center growth and success.

Job Requirements

  • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree required.
  • Board certification in Internal Medicine, Family Medicine, or Geriatrics mandatory.
  • Active state medical license or eligibility to obtain one within the hiring timeframe.
  • Proficiency in managing patient care using electronic medical records systems.
  • Experience with value-based care models and patient outcome accountability preferred.
  • Strong leadership and communication skills to guide care teams effectively.
  • Basic Life Support (BLS) certification required within 90 days of employment.
  • Commitment to serving underserved communities and addressing health disparities.
