Key Responsibilities & Duties

- Engage with hospitalists for continuity of care during hospital admissions.

Job Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree required.
- Board certification in Internal Medicine, Family Medicine, or Geriatrics mandatory.
- Active state medical license or eligibility to obtain one within the hiring timeframe.
- Proficiency in managing patient care using electronic medical records systems.
- Experience with value-based care models and patient outcome accountability preferred.
- Strong leadership and communication skills to guide care teams effectively.
- Basic Life Support (BLS) certification required within 90 days of employment.
