Overview
- Lead and mentor nursing staff in providing exceptional hospice and palliative care services.
- Ensure compliance with program philosophy, goals, and policies while maintaining high-quality care standards.
- Coordinate staff assignments in alignment with established staffing ratios and program requirements.
- Provide guidance on pain and symptom management to empower RN Case Managers.
- Evaluate nursing documentation for completeness and compliance with regulations and policies.
- Collaborate with interdisciplinary teams to enhance patient and family care goals.
- Participate in community outreach presentations to promote hospice services.
- Support the Associate Director in achieving organizational objectives and mentoring staff.
Key Responsibilities & Duties
- Supervise and mentor nursing staff to ensure high-quality hospice and palliative care delivery.
- Develop and manage staff schedules to meet patient care needs effectively.
- Monitor and evaluate nursing documentation for accuracy and compliance.
- Provide leadership in pain and symptom management practices.
- Collaborate with interdisciplinary teams to achieve patient-centered care goals.
- Conduct community presentations to promote hospice services and educate stakeholders.
- Ensure adherence to state and federal regulations in all nursing practices.
- Support organizational objectives through effective mentorship and leadership.
Job Requirements
- Bachelor of Science in Nursing (BSN) or equivalent degree required.
- Minimum of five years of hospice nursing experience.
- Supervisory or leadership experience preferred.
- New York State RN licensure required.
- National Board Certification in Hospice and Palliative Care (CHPN) preferred or obtainable within one year.
- Proficiency in Word, Outlook, and Windows applications.
- Strong ability to supervise and mentor nursing staff effectively.
- Experience working within interdisciplinary team environments.
