Overview

  • Provide compassionate psychosocial support to hospice patients and families, ensuring dignity and comfort during challenging times.
  • Collaborate with interdisciplinary teams to create personalized care plans addressing patient and family needs.
  • Engage in diverse care settings, including inpatient facilities, nursing homes, and home environments.
  • Deliver pre- and post-bereavement counseling, fostering emotional resilience and healing.

Key Responsibilities & Duties

  • Develop and implement tailored social work care plans for hospice patients.
  • Collaborate with interdisciplinary teams to ensure holistic and patient-centered care.
  • Provide counseling and emotional support to patients and their families.
  • Facilitate discharge planning and transitions, ensuring continuity of care.
  • Act as a liaison between hospice staff and external care providers.
  • Maintain accurate clinical documentation and proficiently manage data systems.

  • Ensure compliance with professional standards and state licensure requirements.

Job Requirements

  • Master of Social Work degree from an accredited institution.
  • Active LMSW or LCSW licensure in New York State.
  • Minimum of one year of healthcare social work experience required.
  • Proficiency in Microsoft Word and Excel is preferred.
  • Strong organizational, communication, and interpersonal skills.
  • Experience in hospice or palliative care settings is advantageous.
