Key Responsibilities & Duties

Job Requirements

  • Minimum of two years’ experience in acute care or related healthcare settings.
  • NYS SCREEN Certification within three months of employment.
  • Proficiency in discharge planning and care coordination processes.
  • Exceptional documentation and communication skills for effective collaboration.
  • Ability to work on-site with flexibility for weekends and holidays.
