Job Description

Overview

  • Provide comprehensive nursing care and case management for individuals under OPWDD services, ensuring quality and compliance.
  • Develop and implement individualized care plans tailored to residents' unique health needs and goals.
  • Collaborate with interdisciplinary teams to enhance service delivery and care coordination.
  • Supervise and train healthcare staff, ensuring adherence to nursing standards and regulations.
  • Conduct regular health assessments and monitor treatment efficacy for optimal outcomes.
  • Utilize electronic health record systems for accurate documentation and communication.
  • Participate in team meetings to contribute to strategic care planning and implementation.
  • Ensure compliance with OPWDD regulations and maintain up-to-date medical records.

Key Responsibilities & Duties

  • Perform comprehensive nursing assessments and update care plans as resident conditions evolve.
  • Coordinate health services and monitor residents' progress and treatment effectiveness.
  • Supervise and evaluate healthcare staff, ensuring high-quality care delivery.
  • Provide training on health measures, medication administration, and client-specific medical issues.
  • Maintain accurate documentation of training sessions and staff evaluations.
  • Conduct annual performance evaluations for unlicensed staff involved in direct care.
  • Remain available for on-call support and emergency situations as needed.
  • Collaborate with external service providers to ensure seamless care coordination.

Job Requirements

  • Bachelor of Science in Nursing (BSN) with active RN licensure required.
  • Minimum of 1 year of nursing experience, preferably with OPWDD populations.
  • Proficiency in electronic health record systems and care coordination platforms.
  • Strong knowledge of OPWDD regulations and compliance standards.
  • Ability to supervise and train healthcare staff effectively.
  • Flexibility to adapt to scheduling needs and provide on-call support.
