Overview

• Provide compassionate primary care services to youth in foster care and group residences, addressing physical and mental health needs.

Key Responsibilities & Duties

• Conduct comprehensive medical assessments, histories, and physical exams, emphasizing preventative care and screenings.
• Order and interpret diagnostic tests, formulating treatment plans based on clinical guidelines and results.
• Prescribe medications and recommend therapeutic interventions for acute and chronic conditions.
• Educate clients on health conditions, preventative measures, and therapeutic options to promote wellness.
• Collaborate with case managers, social workers, and psychiatric providers to integrate mental and physical health care.
• Maintain detailed and accurate medical records using electronic health systems.
• Participate in practice improvement projects to enhance healthcare delivery and outcomes.
• Provide tailored health counseling and education, including sexual and reproductive health guidance.

Job Requirements

• Hold a valid Family Nurse Practitioner (FNP) certification and relevant licensure.
• Minimum of one year of clinical experience, preferably in independent practice settings.
• Proficiency in trauma-informed care, harm reduction, and motivational interviewing techniques.
• Experience with electronic health records systems and accurate medical documentation.
• Strong understanding of the interplay between mental and physical health conditions.
