Overview
- Provide comprehensive primary care services to community-based members in collaboration with interdisciplinary teams.
- Manage a caseload of 40-50 patients, ensuring personalized care and health outcome goals.
- Perform multifactorial assessments, including physical examinations and psychosocial evaluations.
- Develop care plans and provide self-management education to patients.
- Work in the field visiting patients at home and remotely from home, with occasional office meetings.
- Act as a clinical resource to members, families, and interdisciplinary teams.
- Support preventive care services and ensure access to necessary resources.
- Contribute to the interdisciplinary co-management program for Elderplan members.
Key Responsibilities & Duties
- Conduct thorough assessments and examinations to address complex patient needs.
- Coordinate and provide primary care services to at-risk community members.
- Develop and implement personalized care plans aligned with health objectives.
- Educate patients on self-management techniques for chronic conditions.
- Collaborate with interdisciplinary teams to optimize patient care outcomes.
- Maintain accurate documentation and ensure compliance with healthcare standards.
- Provide clinical guidance and support to patients and their families.
- Participate in monthly staff meetings and contribute to team development.
Job Requirements
- Master's Degree in Nursing and graduation from an accredited Nurse Practitioner program.
- Minimum one year of clinical experience as a Nurse Practitioner, preferably in geriatrics.
- Prior RN experience in acute, subacute, or long-term care settings preferred.
- Managed care experience is a plus.
- NYS RN/NP License, Registration, and DEA certificates required.
- Strong skills in assessment, diagnosis, treatment, and follow-up care.
- Ability to work independently and collaboratively within interdisciplinary teams.
- Excellent communication and relationship-building skills with patients and families.
