Job Description

Overview

  • Provide exceptional hospice care during on-call hours, addressing urgent patient needs and ensuring quality care delivery.
  • Coordinate with the Call Center Triage and On-Call Manager to establish effective care plans for patients.
  • Respond promptly to patient triage requests, offering both remote and field support as required.
  • Deliver emotional support and guidance to families during patient transitions and end-of-life care.
  • Work within all boroughs, utilizing provided transportation for field visits.
  • Ensure compliance with New York State Standards for Professional Registered Nurses.
  • Utilize home office setup for remote triage and documentation tasks.
  • Collaborate with hospice physicians and the primary care team to optimize patient outcomes.
  • Educate patients and caregivers on care plans, medication management, and safety protocols.

Key Responsibilities & Duties

  • Perform scheduled and unscheduled patient visits during on-call shifts to address urgent physical and psycho-social needs.
  • Communicate effectively with the on-call team to coordinate patient care and address immediate concerns.
  • Implement and adjust care plans based on patient conditions and triage assessments.
  • Provide emotional support to families during patient transitions and end-of-life care.
  • Document patient interactions and care adjustments accurately in compliance with standards.
  • Attend patient deaths as directed, offering guidance and support to families.
  • Collaborate with hospice physicians to address urgent patient needs and establish safe care plans.
  • Utilize transportation services for field visits across boroughs as required.
  • Maintain compliance with New York State Standards for Professional Registered Nurses.

Job Requirements

  • Graduate from an accredited School of Nursing with an Associate of Applied Science degree.
  • Minimum of two years' experience in Medical-Surgical Nursing, Community Health, Hospice, or Oncology nursing.
  • Preferred four years of relevant nursing experience.
  • Active New York State RN licensure and valid driver's license preferred.
  • Commitment to sit for the national board certification CHPN within one year of hire.
  • Proficiency in Word, Excel, and Windows applications.
  • Ability to work hybrid, combining remote triage and field visits.
  • Strong communication skills for collaboration with hospice physicians and care teams.
  • Knowledge of New York State Standards for Professional Registered Nurses.
