Job Description

Overview

  • Provide comprehensive case management services to patients, ensuring optimal care coordination and resource utilization.
  • Collaborate with healthcare teams to develop and implement individualized care plans for patients.
  • Ensure compliance with healthcare regulations and standards in all case management activities.
  • Support inpatient care by managing transitions and discharge planning effectively.
  • Utilize clinical expertise to assess patient needs and recommend appropriate interventions.
  • Maintain accurate and timely documentation of case management activities and patient progress.
  • Act as a liaison between patients, families, and healthcare providers to facilitate communication and care delivery.

Job Requirements
