Registered Nurse Case Manager

Overview

• Provide comprehensive case management services during an 8-week contract assignment.
• Work on-site in a healthcare facility during 10-hour day shifts.

Key Responsibilities & Duties

• Conduct patient assessments to determine care needs and develop plans.
• Coordinate with healthcare professionals to implement patient care plans.
• Monitor patient progress and adjust care plans as necessary.
• Educate patients and families about care options and resources.
• Ensure compliance with healthcare regulations and standards.
• Document patient care activities accurately and timely.
• Participate in team meetings to discuss patient care strategies.
• Provide support during patient transitions between care settings.

Job Requirements

• Associate of Applied Science (AAS) in Nursing required.
• Minimum of 2 years of case management experience preferred.
• Active Registered Nurse (RN) license required.
• Strong knowledge of healthcare regulations and standards.
• Excellent communication and interpersonal skills.
• Ability to work effectively in a team-oriented environment.
• Proficiency in patient care documentation and record-keeping.
