Overview
- Provide dedicated social work services to residents, addressing diverse needs including mental health, memory care, and domestic challenges.
- Collaborate with multidisciplinary teams to ensure comprehensive support and advocacy for resident well-being.
- Engage with families and external organizations to coordinate tailored assistance and resources.
- Organize community events to promote mental and physical health, social skills, and educational opportunities.
- Conduct home visits and liaise with healthcare providers to assess and address individual needs.
- Maintain detailed records and provide regular updates to the Resident Relations team.
- Utilize office equipment and technology to perform onsite duties effectively.
Key Responsibilities & Duties
- Manage cases involving residents facing mental health challenges, emotional disturbances, and domestic issues.
- Provide referrals to appropriate services, including healthcare providers and community resources.
- Facilitate discussions and activities at the Community Center to enhance resident engagement and skills.
- Support Community Center operations and collaborate with the management team for effective programming.
- Advocate for residents by coordinating with internal teams and external organizations.
- Conduct home visits to assess living conditions and provide necessary interventions.
- Communicate with families and healthcare providers to ensure continuity of care.
- Prepare comprehensive reports and statistics to inform decision-making and program improvements.
Job Requirements
- Master of Social Work degree and LMSW certification required.
- Minimum of 2 years of professional experience in social work or related fields.
- Proficiency in computer applications such as Outlook, Asana, Word, and Teams.
- Strong interpersonal skills and ability to communicate effectively with diverse populations.
- Experience in case management and advocacy for individuals with complex needs.
- Ability to perform onsite duties effectively using provided office equipment.
- Knowledge of community resources and ability to establish connections with external agencies.
