Overview
- Serve as a vital link between medical services and residents, ensuring comprehensive nursing care and adherence to healthcare standards.
- Provide direct nursing care and perform routine medical procedures within the scope of nursing practice.
- Supervise and support LPNs, AMAP, and Direct Support Professionals in nursing tasks and activities.
- Develop and implement individualized nursing care plans, updating them annually or as needed.
- Coordinate acute and ongoing health services, monitoring health status and prescribed treatment efficacy.
- Conduct regular nursing assessments and maintain accurate medical record documentation.
- Provide training to staff on health measures, medication administration, and client-specific medical issues.
- Participate actively in interdisciplinary team meetings and quality improvement projects.
Key Responsibilities & Duties
- Deliver nursing care as prescribed, ensuring compliance with recognized standards and regulations.
- Perform physical examinations and assessments for program participants upon entry and quarterly thereafter.
- Coordinate health services and communicate findings to clinical personnel effectively.
- Train staff on health methodologies, communicable disease control, and client-specific medical issues.
- Evaluate clinical performance of direct support staff annually, ensuring adherence to procedures.
- Maintain documentation of training curriculums, attendance, and testing outcomes.
- Utilize electronic health record systems for care coordination and documentation management.
- Support agency initiatives to enhance patient-centered care and confidentiality.
Job Requirements
- Active, unrestricted New York State license as a Registered Professional Nurse.
- Bachelor’s degree in Nursing (BSN) and minimum 3 years of clinical experience preferred.
- Experience with intellectually disabled populations and managerial roles preferred.
- Proficiency in electronic health record systems and care coordination tools.
- Strong verbal and written communication skills with the ability to work independently.
- Sound judgment and decision-making capabilities in a team-oriented environment.
- Knowledge of OPWDD regulations and healthcare policies.
