Job Description

Overview

  • Serve as a vital link between medical services and program participants, ensuring comprehensive nursing care and adherence to regulatory standards.
  • Provide direct nursing care, perform assessments, and develop individualized care plans tailored to each participant's needs.
  • Supervise and train Licensed Practical Nurses (LPNs) and Direct Support Professionals in nursing tasks and preventive health measures.
  • Coordinate acute and ongoing health services while monitoring health status, lab results, and treatment efficacy.
  • Ensure accurate and timely medical record documentation, reflecting outcomes and nursing interventions.
  • Conduct annual performance evaluations for staff, focusing on clinical competencies and adherence to protocols.
  • Participate actively in interdisciplinary team meetings to enhance care coordination and service delivery.
  • Maintain proficiency in electronic health record systems for effective care coordination and documentation.

Key Responsibilities & Duties

  • Provide nursing care as prescribed, ensuring compliance with standards and regulations.
  • Perform regular physical examinations and assessments of program participants.
  • Develop and update individualized nursing care plans annually or as needed.
  • Train staff in health and hygiene practices, communicable disease control, and emergency first aid.
  • Supervise LPNs and direct support staff, ensuring adherence to nursing protocols.
  • Coordinate health services and communicate findings to clinical personnel.
  • Monitor and document health status, lab results, and treatment outcomes.
  • Conduct training sessions for staff on medication administration and specialized care techniques.
  • Participate in interdisciplinary team meetings to optimize care delivery.

Job Requirements

  • Associate of Applied Science (AAS) degree in Nursing or related field.
  • Minimum of 1 year of experience in nursing, preferably with OPWDD or MRDD populations.
  • Proficiency in electronic health record management and care coordination systems.
  • Strong knowledge of OPWDD regulations and nursing standards.
  • Ability to supervise and train staff in clinical procedures and health practices.
  • Effective communication skills for interdisciplinary team collaboration.
  • Flexibility in work hours and occasional on-call availability.
