Overview

• Provide essential healthcare services as a Licensed Practical Nurse (LPN) within a dynamic and supportive environment.
• Collaborate with interdisciplinary teams to address clients' medical and psychosocial needs effectively.
• Conduct health and vitals checks, ensuring optimal client outcomes through proactive care management.
• Maintain accurate medical records, including medication administration logs and controlled substance documentation.
• Engage directly with clients to provide health education and advocate for their health preferences.
• Participate in treatment team meetings and case conferences to share insights and coordinate care plans.
• Offer emergency first aid and CPR assistance as needed, ensuring safety and well-being.
• Contribute to a positive and professional work environment through collaboration and dedication.

Key Responsibilities & Duties

• Supervise assigned staff, ensuring adherence to healthcare standards and protocols.
• Coordinate referrals and suggestions for medical treatment and psychiatric rehabilitation.
• Act as primary medical case manager, managing client health needs comprehensively.
• Document and maintain accurate records of client care and medication administration.
• Provide health education to clients, promoting informed decision-making and goal setting.
• Collaborate with treating physicians, pharmacies, and other healthcare professionals to optimize client care.
• Ensure adequate medication supply and follow up on client needs proactively.
• Perform emergency first aid and CPR assistance when required.

Job Requirements

• Active Licensed Practical Nurse (LPN) certification is mandatory.
• Associate of Science (AS) degree in Nursing or related field required.
• Minimum of 1 year of relevant healthcare experience; 2 years preferred.
• Strong knowledge of medical case management and healthcare protocols.
• Proficiency in maintaining accurate medical records and documentation.
• Excellent communication and collaboration skills within interdisciplinary teams.
• Capability to provide emergency first aid and CPR assistance effectively.
