Job Description

Overview

  • Provide social work services to participants and caregivers in an Enhanced Memory Impaired program.
  • Work on-site with flexible hours, including alternating Saturdays, in a supportive environment.
  • Contribute to assessments and develop individualized service plans for participants.
  • Assist participants and caregivers in accessing community resources and support.
  • Coordinate discharge plans and ensure smooth transitions for participants.
  • Maintain accurate documentation of services provided to ensure compliance.
  • Collaborate with a multidisciplinary team to enhance dementia care services.

Key Responsibilities & Duties

  • Conduct assessments and develop personalized service plans for participants.
  • Provide emotional and practical support to participants and caregivers.
  • Assist in accessing community resources and services for enhanced care.
  • Coordinate discharge planning and transitions for participants.
  • Maintain detailed and accurate documentation of all services provided.
  • Collaborate with team members to ensure high-quality dementia care services.
  • Participate in program development and improvement initiatives.
  • Provide bilingual support in Spanish, if applicable, to enhance communication.

Job Requirements

  • Master of Social Work (MSW) degree with LMSW licensure.
  • Minimum of 1 year experience, with 5 years preferred, in dementia care.
  • Certification in dementia care is a plus.
  • Bilingual proficiency in Spanish is highly desirable.
  • Strong documentation and record-keeping skills.
  • Ability to work on-site with flexible hours, including alternating Saturdays.
