Overview

- Promote collaboration between hospital and hospice teams for comprehensive patient care.
Key Responsibilities & Duties
- Supervise interdisciplinary teams, providing clinical guidance and operational support to ensure high-quality care delivery.
- Conduct regular team meetings, rounds, and individual supervision to maintain care standards.
- Monitor staffing utilization and implement strategies to optimize team performance and efficiency.
- Ensure compliance with federal and state regulations governing hospice care practices.
- Lead quality improvement initiatives to enhance patient care and team effectiveness.
- Provide education and training to staff on hospice principles and symptom management techniques.
- Collaborate with business development teams to identify and pursue growth opportunities.
- Participate in strategic planning and policy development for hospice care delivery.
Job Requirements
- Bachelor of Science in Nursing (BSN) required; MSN or Master’s degree preferred.
- Minimum of two years of hospice nursing experience required; four years preferred.
- Supervisory or leadership experience in healthcare settings is highly desirable.
- CHPN certification required or to be obtained within one year of hire.
- Strong knowledge of hospice regulations, patient care standards, and symptom management practices.
- Valid New York State RN licensure and CPR-BLS certification are mandatory.
- Ability to manage priorities effectively and work flexible hours in a hybrid environment.
- Valid NYS Driver's license preferred for travel between locations.
