Key Responsibilities & Duties
- Supervise and support interdisciplinary hospice teams, ensuring compliance with care standards and regulatory requirements.
- Manage team operations including hiring, scheduling, performance evaluations, and professional development.
- Conduct facility rounds, providing direct oversight and assistance to staff members.
- Lead interdisciplinary team meetings to review patient care plans and ensure optimal treatment strategies.
- Develop and implement quality improvement projects to enhance team performance and patient care.
- Collaborate with business development teams to identify growth opportunities and strengthen referral networks.
- Ensure compliance with CMS, state, and accrediting body regulations for hospice care delivery.
Job Requirements
- Bachelor of Science in Nursing (BSN) required; Master’s degree preferred for advanced leadership roles.
- Minimum of two years of hospice nursing experience; supervisory experience preferred for team management.
- CHPN certification required or to be obtained within one year of hire.
- Strong knowledge of hospice regulations, symptom management, and patient care standards.
- New York State RN licensure and CPR-BLS certification required for clinical practice.
- Valid NYS driver’s license preferred for travel between healthcare facilities.
- Proven leadership skills with the ability to manage priorities and foster team collaboration.
