Job Description

Overview

  • Provide comprehensive nursing care to clients with mental health and substance issues in a supportive environment.
  • Work on-site at a facility dedicated to rehabilitation and client care.
  • Collaborate with a multidisciplinary team to address clients' medical and psychosocial needs.
  • Ensure proper documentation and compliance with medication administration protocols.
  • Advocate for client health preferences and goals through education and partnership.
  • Perform health checks and coordinate with nursing leadership for optimal outcomes.
  • Provide emergency first aid and CPR assistance when required.
  • Participate in treatment team meetings and case conferences.

Key Responsibilities & Duties

  • Supervise assigned staff and oversee client care activities.
  • Link clients to medical treatments and psychosocial rehabilitation services.
  • Coordinate information with physicians, pharmacies, and other treatment providers.
  • Manage client health needs through regular health and vitals checks.
  • Document and maintain medication administration records and controlled substance logs.
  • Educate clients on health practices and advocate for their health goals.
  • Provide emergency medical assistance as needed.
  • Perform related duties as assigned to support facility operations.

Job Requirements

  • Active Licensed Practical Nurse (LPN) certification is required.
  • Associate of Science (AS) degree in nursing or related field.
  • Minimum of 1 year of nursing experience, preferably 2 years.
  • Experience working with clients with mental health and substance issues is preferred.
  • Strong documentation and organizational skills.
  • Knowledge of medication administration protocols and compliance standards.
  • Ability to provide health education and advocate for client preferences.
  • Proficiency in emergency medical assistance and CPR.
