Overview
- Coordinate and implement home care plans for patients, ensuring comprehensive and interdisciplinary care.
- Evaluate patient needs and update care plans in collaboration with physicians and caregivers.
- Provide bedside nursing care and conduct initial assessments for home health services.
- Work with discharged patients transitioning to home care, addressing acute needs.
- Ensure positive clinical outcomes and minimize risks of rehospitalization through effective case management.
- Collaborate with family members and professional caregivers for optimal patient care.
- Field-based role requiring travel within the Bronx region; car and driver's license required.
- Full-time position with weekday hours and on-call weekend duties once a month.
Key Responsibilities & Duties
- Conduct initial patient assessments and develop individualized home health plans of care.
- Provide direct bedside nursing care to patients with acute needs.
- Coordinate patient care activities and communicate findings with caregivers and family members.
- Case manage assigned patients to ensure effective care delivery and positive outcomes.
- Review patient insurance information and ensure compliance with care plans.
- Collaborate with physicians to update and maintain patient care plans.
- Participate in on-call duties once a month to provide weekend coverage.
- Maintain accurate and timely documentation of patient care activities.
Job Requirements
- Associate of Applied Science (AAS) degree required; Bachelor of Science in Nursing (BSN) strongly preferred.
- Registered Nurse (RN) certification required.
- Minimum of 2 years of experience in hospital, acute, med/surg, COC, CHHA, or nursing home settings.
- Field-based role requiring 100% travel within the Bronx region; valid driver's license and car required.
- Ability to pass a pharmacy test as part of the interview process.
- Strong case management skills to ensure effective care delivery and minimize rehospitalization risks.
- Excellent communication skills to collaborate with physicians, caregivers, and family members.
