Job Description

Overview

  • Coordinate and implement comprehensive home care plans for patients in Manhattan.
  • Evaluate patient needs and update care plans in collaboration with physicians.
  • Ensure effective communication with families and professional caregivers.
  • Provide field-based care with minimal office requirements.
  • Work on-call one weekend per month to support patient needs.
  • Contribute to positive clinical outcomes and reduced rehospitalization risks.
  • Collaborate with interdisciplinary teams to deliver high-quality patient care.

Key Responsibilities & Duties

  • Develop and update patient-centered home health plans of care.
  • Coordinate activities and services to meet individual care plans.
  • Review patient insurance information and ensure proper documentation.
  • Case manage assigned patients to achieve clinical goals.
  • Perform fieldwork in Manhattan with 100% territory coverage.
  • Collaborate with physicians and interdisciplinary teams for effective care delivery.
  • Provide on-call support one weekend per month.
  • Ensure compliance with healthcare regulations and standards.

Job Requirements

  • Associate of Applied Science (AAS) in Nursing or related field.
  • Registered Nurse (RN) certification required.
  • Minimum of 1 year medical-surgical or home care experience preferred.
  • New graduate RNs will be considered.
  • Strong case management and care coordination skills.
  • Ability to work independently in a field-based role.
