Job Description

Overview

  • Provide psychosocial support and social work services to hospice patients and families in various care settings.
  • Collaborate with interdisciplinary teams to ensure comprehensive patient care plans are developed and implemented effectively.
  • Develop and execute social work plans of care and discharge plans for hospice patients.
  • Offer pre and post-bereavement support to families, ensuring compassionate and professional assistance.
  • Act as a liaison between hospice staff and inpatient or nursing home personnel to coordinate care.
  • Provide supervision hours for LCSW certification, supporting professional development.
  • Maintain accurate and timely documentation of clinical data and patient interactions.
  • Participate in interdisciplinary team meetings to discuss patient progress and care strategies.
  • Work primarily in Manhattan field settings, with occasional weekend coverage as needed.

Key Responsibilities & Duties

  • Conduct psychosocial assessments and develop individualized care plans for hospice patients.
  • Provide emotional and practical support to patients and families during end-of-life care.
  • Collaborate with healthcare teams to ensure seamless care transitions and discharge planning.
  • Facilitate communication between patients, families, and care providers to address concerns and needs.
  • Offer bereavement counseling and support to families following patient loss.
  • Document patient interactions and care plans accurately using clinical data systems.
  • Provide supervision hours for LCSW certification to eligible candidates.
  • Participate in virtual and in-person interviews as part of the hiring process.
  • Ensure compliance with hospice care standards and regulations in all activities.

Job Requirements

  • Master of Social Work degree from an accredited institution is required.
  • New York State LMSW or LCSW licensure is mandatory.
  • Minimum of one year of experience in a healthcare social work setting.
  • Preferred four years of experience, including hospice or palliative care internship.
  • Proficiency in Microsoft Word and Excel is advantageous.
  • Strong organizational, communication, and listening skills are essential.
  • Ability to work effectively within an interdisciplinary team environment.
  • Fieldwork in Manhattan is required, with occasional weekend coverage.
