Overview
- Provide psychosocial support and social work services to hospice patients and families in various care settings.
- Collaborate with interdisciplinary teams to ensure comprehensive patient care plans are developed and implemented effectively.
- Develop and execute social work plans of care and discharge plans for hospice patients.
- Offer pre and post-bereavement support to families, ensuring compassionate and professional assistance.
- Act as a liaison between hospice staff and inpatient or nursing home personnel to coordinate care.
- Provide supervision hours for LCSW certification, supporting professional development.
- Maintain accurate and timely documentation of clinical data and patient interactions.
- Participate in interdisciplinary team meetings to discuss patient progress and care strategies.
- Work primarily in Manhattan field settings, with occasional weekend coverage as needed.
Key Responsibilities & Duties
- Conduct psychosocial assessments and develop individualized care plans for hospice patients.
- Provide emotional and practical support to patients and families during end-of-life care.
- Collaborate with healthcare teams to ensure seamless care transitions and discharge planning.
- Facilitate communication between patients, families, and care providers to address concerns and needs.
- Offer bereavement counseling and support to families following patient loss.
- Document patient interactions and care plans accurately using clinical data systems.
- Provide supervision hours for LCSW certification to eligible candidates.
- Participate in virtual and in-person interviews as part of the hiring process.
- Ensure compliance with hospice care standards and regulations in all activities.
Job Requirements
- Master of Social Work degree from an accredited institution is required.
- New York State LMSW or LCSW licensure is mandatory.
- Minimum of one year of experience in a healthcare social work setting.
- Preferred four years of experience, including hospice or palliative care internship.
- Proficiency in Microsoft Word and Excel is advantageous.
- Strong organizational, communication, and listening skills are essential.
- Ability to work effectively within an interdisciplinary team environment.
