Job Description

Overview

  • Provide comprehensive medical case management to clients in supportive housing, addressing complex health and psychiatric needs with personalized care plans.
  • Collaborate with multidisciplinary teams to ensure optimal client outcomes and continuity of care through effective communication and coordination.
  • Perform routine health assessments, document findings, and maintain accurate medication administration records for compliance and safety.
  • Educate clients on health topics, empowering them to make informed decisions and advocate for their well-being.
  • Assist clients in navigating healthcare services, resources, and administrative tasks to support their independence and quality of life.
  • Provide on-site support during designated hours, including weekends, ensuring consistent availability for client needs.
  • Administer emergency first aid and CPR as required, maintaining readiness for urgent situations.
  • Participate in team meetings and case conferences to share insights and contribute to coordinated care strategies.

Key Responsibilities & Duties

  • Supervise assigned staff, ensuring adherence to medical protocols and fostering a collaborative work environment.

Job Requirements

  • Valid Licensed Practical Nurse (LPN) certification is mandatory.
  • Associate of Science (AS) degree in nursing or a related field is required.
  • Minimum of 1 year of professional experience, with 3 years preferred.
  • Proficiency in medical case management and psychiatric rehabilitation practices.
  • Strong documentation skills for maintaining client records and medication logs.
  • Ability to educate and advocate for clients, promoting their health goals effectively.
  • Experience with emergency response procedures, including first aid and CPR.
  • Excellent communication and organizational skills to manage diverse responsibilities.
